I am located in the middle of the large plaza on the corner of Old Nashville Hwy and Hazelwood Dr behind the 

Walgreens.

 Park by the two staircases and enter through the door in between the staircases. Use the elevator to come up to the second floor. I am in Suite 210.

There is a restroom to the right as you get off the elevator if you need to use it before your appointment.
